All applications must include a project summary following the format below.  The project summary must not exceed one page.  Should your project be recommended for funding, the Project Summary will be given to the State Library Board as an introduction to your project.  Two sample project summaries can be found after the format guidelines.

RIVER PUBLIC LIBRARY

Summary statement:  Federal funds will be used to purchase books, magazines, and CDs to establish a Spanish-language collection for this growing segment of the community.

County or counties covered by the project:  River County in East Central Ohio.

Targeted population: 
283 Hispanic families within the county.  Indicators show that this population will continue to increase.

Need for the project: 
The purpose of this project is to develop a circulating collection of new Spanish-language materials and English-as-a Second –Language materials to meet the needs of the growing Hispanic population in River County.  The library currently has a very small collection that is well used, but providing a larger collection will make a bigger impact and allow the library to serve this population better.  The library has seen a greater number of Hispanics using the library.  The Spanish-language Internet and computer workstation is regularly used by Hispanics.  This project will also assist the library to leverage partnerships with others in the community who are working with the Hispanic population.

Objectives of the proposal:
1.  To establish a circulating collection with a minimum of 342 Spanish-language materials in a variety of formats for all age groups.
2.  To market the new collection and other services to the Latino population.
3.  To explore a minimum of 3 outreach opportunities in the county at locations where a large portion of the Hispanic population works or congregates.

Method of Evaluation and Intended Impact on the Targeted Population:
The circulation code will indicate if at least 50% of the collection circulates during the first year and if circulation of the entire collection grows by at least 10% each succeeding year.  Patrons will be consulted formally and informally to identify future needs and purchases.  Having a Spanish-language collection and outreach efforts will make the library a welcoming place to Hispanics in the community and one where they can go to have their informational and recreational needs met.

OHIO COUNTY DISTRICT PUBLIC LIBRARY

Summary statement:  Federal funds will be used to purchase website redesign services, 10 computers with large monitors and peripherals loaded with a variety of software for those with disabilities, and ADA-compliant furniture and to contract for staff awareness training and sign-language training.  These items will be used to initiate a program at the library’s ten service outlets to provide library services to those with disabilities.

County or counties covered by the project:  Ohio County is located in Southwest Ohio.

Targeted population: 
This project targets adults with mobility, hearing and visual disabilities.  Out of the 38,010 adults with disabilities in the county, 78% fall into this population segment.

Need for the project:
The Library currently has very limited services and resources for adults with mobility, hearing and visual disabilities.  The primary purpose of this project is to provide the mobility, hearing and visually disabled adults in the county with increased access to a wider array of library resource, programs, and services.  The project’s secondary purpose is to train library staff members to work more effectively with these adults as well as the technology specifically targeted for those users.  The Library has worked with various county agencies that serve this segment of the population to develop a plan to meet identified library service needs of this population.  A Planning Committee was formed from these agencies as well as representatives from the disabled population.  The Committee conducted individual interviews and focus groups with various organizations.  Results indicated gaps in library services such as outreach programs, library programs done in house specifically for the target population and library collections that contained more formats of interest to this clientele.  Another gap identified in existing library services was the need for adaptive technology for adults with mobility hearing and visual impairments.  The community representatives also expressed the need for a library staff trained both to work in a knowledgeable manner with their clients and to create a welcoming environment tin the library. 

Objectives of the proposal:
1.  To upgrade the Library’s website.
2.  To install ADA compatible workstations in each branch which will be used by the target population 50% of the time in which they are in use.
3.  To train 100% of library staff to communicate and work effectively with the target population.
4.  To expand resources for the target population with circulation of these formats increasing by 20% in year one. 
5. To expand library outreach to senior centers by 40% (from 30 to 42) programs per year and to have a minimum of 2 programs per month in each branch location aimed at the target audience. 

Method of Evaluation and Intended Impact on the Targeted Population:
The project will be evaluated based on the target population’s increased use of the library, its resources, equipment and programs.  Surveys of the target population and changes in staff behavior will assess impact the project is having on the library, the targeted population and the community.
